Consumer Participation
As part of the Bridges to Excellence (BTE) initiative, there are two consumer web-based tools - Diabetes CareRewards and HealthGrades Rating site.
The HealthGrades tool allows individuals to view recognized physicians as part of the overall BTE program and rate their experience of care. The Diabetes CareRewards program is designed to encourage patients to take a more active role in managing chronic illnesses through an on-line customized care management tool. This innovative tool allows individuals to record self-care activities, learn more about their illness, and earn reward points toward a range of incentives.
HealthGrades and Bridges to Excellence (BTE) share a common mission of improving quality and patient outcomes nationwide. Our collective efforts provide consumers with information to make better healthcare choices in two ways:
