Physicians
participating in the Bridges to Excellence pay-for-performance
(P4P) effort throughout Massachusetts are recognized and rewarded
for delivering higher quality care to their patients. Bridges
to Excellence (BTE) was created to encourage higher quality
care through incentives and is made up of physicians, health
plans and several of The Commonwealth's largest employers,
including General Electric, Verizon, Raytheon, AstraZeneca
and IBM.

Physician
Office Link Program (POL)
The POL program
is designed to promote and reward office practices for the
use of systematic information to enhance the quality of patient
care.
Systematic processes have been shown to contribute to improvements
in health care, including physicians' ability to monitor their
patients' medical histories and their ability to follow up
with their patients and with other providers.
Office practice
sites which implement specific processes to reduce errors
and increase quality can earn up to $50 per year for each
patient covered by a participating employer.
A report card for each physician office describing its performance
on the program measures is made available to the public.
Diabetes
Care Link Program (DCL)
The DCL program
is focused on improving the quality of care for patients with
diabetes.
Patients who use disease management tools to self-manage their
own diabetes care can earn rewards for achieving target goals
that improve their health.
Physicians who demonstrate they are top performers in diabetes
care can earn up to $80 per year for each diabetes patient
covered by a participating employer.
Cardiac
Care Link Program (CCL)
The CCL program
is focused on improving the quality of care for patients with
cardiovascular disease.
Patients who use
disease management tools to self-manage their own cardiac
care can earn rewards for achieving target goals that improve
their health.
Physicians who demonstrate they are top performers in cardiac
care can earn up to $160 per year for each cardiac patient
covered by a participating employer.

Administrator Responsibilities
- Obtain licensing
from BTE
- Perform the
administrative functions necessary to identify eligible
physicians and make incentive payments to those treating
diabetic patients with insurance coverage provided by a
participating employer.

from communications designed by participating employers
to educate employees about certified physicians and the
benefits of seeking treatment from such physicians.
